Sandlot Solutions expanded its reach in the health plan market, as it was recently selected by South Country Health Alliance, a county-based health plan serving 12 counties in Minnesota, to support South Country’s transition to value-based health care.
South Country will leverage the use of Sandlot’s Connect, Dimensions, and Metrix products and technologies to support the new pay-for-performance contracts. Sandlot Solutions, certified by the State of Minnesota, is one of only four organizations authorized to provide health information exchange services in the state and facilitates data sharing between providers, helping to decrease fragmentation and duplication of health care services.
“In addition to meeting the stringent state requirements, Sandlot Solutions offered the most sound technology solution,” said John Whitington, South Country’s Chief Information Officer. “They stood alone in their ability to provide gaps in care notifications at the point of care with their patent-pending analytics tool and digital reminder system, Metrix. Sandlot’s approach to integration is flexible, allowing providers to integrate into their existing workflow. It offers participants a centralized location to store and manage quality-based measures, including Health Care Effectiveness Data and Information Set (HEDIS), Product Quality Research Institute (PQRI) and Five-Star Quality Measures, specifically.”

Sandlot Solutions and South Country Health Alliance signed a five-year agreement and will begin work together next month. This marks the most recent initiative by Sandlot Solutions to collaborate with Minnesota-based health care organizations since joining the Statewide Shared Services Collaborative in November 2013.
