Changes to Be Rewarded Programs

Preventive care is an important part of your health. South Country Health Alliance rewards members for taking charge of their health and completing important preventive care services on time.

Effective January 2017, we made some changes to our Be Rewarded™ preventive care voucher reward programs.

Here’s what changed:

  • The age requirement for women to receive the mammogram reward went up from age 40 to age 50. Now, women who are age 50 and older on the date of service can receive a $50 gift card for getting a mammogram. This reward is available for each calendar year visit.
  • The adolescent immunization reward now only includes human papillomavirus (HPV) vaccinations. Adolescents who complete the series of HPV vaccinations before age 13 are eligible for a $50 gift card.
  • The young adult well-care visit (C&TC exam) now includes a bonus reward for completing a chlamydia (sexual health) screening. Members ages 18-21 who choose to complete a Chlamydia screening during their C&TC exam can receive a $25 bonus reward in addition to the $50 gift card for completing their well-care visit.

South Country Health Alliance will honor previous vouchers with dates of service on or before January 31, 2017.

All of our vouchers are available to download and print on our website. You can also call Member Services to request a voucher, or talk to your county’s Public Health department. A complete booklet of our vouchers is also available upon request when you call Member Services or your Public Health department.
