  • DIRECTOR OF REPORTING AND ANALYTICS: Lead all aspects of data management, including developing analytics and data strategies and implementing solutions. The strategies will include information access and delivery methods, analytics applications, and tool portfolio.

    Required: Bachelor’s degree in information technology, health informatics, or a related field; minimum 6 years informatics experience; minimum 2 years management experience.

    Hours/Pay Period: Full-time

  • CARE MANAGER: Responsible for completing health assessments in a timely manner, developing and implementing care plans for enrollees, and assisting enrollees in achieving their health goals. This position acts as the care coordinator working with an interdisciplinary care team to manage the care and pharmacotherapy of enrollees.

    Required: Current, valid, and unrestricted RN license in Minnesota or bachelor’s degree in social work or behavioral science; minimum 3 years clinical experience with at least 2 years of that experience in public health, long term care, or other settings that cater to vulnerable adult populations, such as experience working directly with the programs that serve frail members or vulnerable adults.

    Hours/Pay Period: Full-time

    Minimum Salary: $24.69/hr (Non-Exempt)

  • CLINICAL PHARMACIST: Provide consultative service to various departments and staff of South Country Health Alliance related to pharmacy operations, pharmacy utilization management, Medicare (Part D), and Medicaid formulary management to comply with state and federal regulations and DHS contractual obligations; Apply strategies that appropriately manage pharmaceutical cost and assure member access to safe and effective pharmaceuticals.

    Required: Bachelor’s degree in pharmacy (PharmD preferred), with 5 years experience in retail pharmacy, working with Medicare and Medicaid claims; Demonstrated experience in multiple areas such as: Utilization Management (UM), Disease Management (DM), Medication Therapy Management (MTM), Managed Care Organization (MCO), and Pharmacy Benefit Manager (PBM); Current and unrestricted Minnesota pharmacy license.

    Hours/Pay Period: This is a part-time (0.4 FTE) position with the expectation to work 2 days/week (16 hours).

    Minimum Salary: $92,173/annual (based on 1.0 FTE)

  • DIRECTOR OF HEALTH SERVICES: Responsible for directing and managing the overall operations of the Health Services department with specific responsibility for the company’s case management and utilization management activities. Responsible for the development, implementation, and monitoring of SCHA’s integrated care management model, ensuring clinical initiatives are effective and efficiently implemented, policies and procedures are developed and maintained to accurately reflect staff performance expectations, and activities comply with state and federal regulation and DHS contractual obligations.

    Required: Bachelor’s degree in nursing, public health or health care administration; 6 years experience in mid-level management or supervision, and 3 years clinical or public health experience.

    Preferred: Licensed RN in the state of Minnesota.

    Hours/Pay Period: Full-time

    Minimum Salary: $98,499/annual (Exempt)
