South Country Health Alliance’s Step Up! For Better Health disease management program focuses on YOU—not just your disease. Our program is designed to support you in making health decisions in order to live your life well. Here are some things you can learn:
- Getting the treatment and preventative information you need
- Identifying and managing your risks for other conditions
- Understanding your medications and the importance to take them as directed
- Making healthy lifestyle changes to reach your personal health goals
- Understanding and following your treatment plan that your medical practitioner recommends
Step Up! For Better Health supports you with friendly telephone calls and educational mailings from our registered nurses. Based on your health care needs, we can give you individualized support to help you make healthy choices and live well. As a South Country member, you receive this supportive health program as part of your benefits. There is nothing extra to pay, and participating in the program is always up to you!

Disease Management Conditions
South Country’s Step Up! For Better Health can help you learn more about asthma. People with asthma can have severe symptoms and trouble breathing. Others have less severe symptoms. If your practitioner tells you that you have asthma, we will work with you to make sure you know as much as you can about your asthma. Some things the nurse may talk to you about may include an asthma action plan, the importance of medications, asthma triggers, and other information that you tell us is important to you. Call us if your medical practitioner tells you that you have asthma.
South Country’s Step Up! For Better Health can help you learn more about congestive heart failure. Heart failure is not a disease. It is a condition that happens when the heart cannot pump enough blood for your body. There are groups of this heart condition that range from mild to very serious. Everyone with heart failure needs care from a medical practitioner. If you want to learn more about congestive heart failure, call us and talk to one of our nurses.
