Community Well: Member Contacts/Visits

Member Contacts/Visits

All members will have a face-to-face visit at least annually or as directed by the member’s care plan.

Contact with members MUST be attempted at least quarterly. Quarterly contact can be telephonic or face-to-face.

If a care transition occurs, the case manager/care coordinator will reach out to the member within the required timelines to assure care transition requirements are met.
