We are excited to have you as a new member of our health plan.
South Country Health Alliance is a health plan owned and operated by 11 counties in Minnesota: Brown, Dodge, Goodhue, Kanabec, Morrison, Sibley, Steele, Todd, Wabasha, Wadena, and Waseca (also serves Freeborn County). (See our service area.) We are committed to improving health care in rural Minnesota and meeting our members’ health care needs.

Perks & Benefits
South Country Health Alliance has a strong preventive care program that offers up to $100 rewards to you and your family when you seek timely screenings, vaccinations, and well-care visits. Members can also get up to $20 off the cost of your monthly health club membership fees through our Be Fit program. RideConnect offers transportation for eligible members who need rides to and from your covered health care appointments. Non-emergency medical transportation (NEMT) services include unassisted and assisted transportation, depending on your level of need. Learn more about our RideConnect transportation services
Members have access to the Ask Mayo Clinic Nurse Helpline, where you can get free advice from experienced nurses to help you decide what care you need. Mayo Clinic counselors are also available to help you quit smoking once and for all through the EX Program. Learn more about the nurse helpline and tobacco cessation
