Annual health plan selection is taking place now, and you may have a choice of health plans for 2018. South Country Health Alliance wants to be that choice!
You have received a letter from the Minnesota Department of Human Services (DHS) that lists which health plans are available in your county. The letter explains the next steps to take to choose your health plan. Read the letter carefully, and follow the directions.

It’s true. We are owned by 11 counties in Minnesota (service area)
. A county commissioner elected in each of these counties serves on our Joint Powers Board
and leads our health plan. Together, they make decisions based on the input and best interests of the people who live in their counties. So really, we work for YOU.
As a South Country Health Alliance member, you’ll have access to a broad network of health care providers
, both inside and outside of your county. You also have access to an established network of dentists, chiropractors, mental health specialists, pharmacies, and many other types of health care providers throughout the state.

South Country Health Alliance understands the local communities in our service area. Each county has a team who works together to connect you with the local providers and resources that will give you the care you need. Unlike other health plans, South Country Health Alliance has a Community Care Connector
in each county who works with members right in their own community.
We want to prevent health problems before they occur. South Country Health Alliance has a strong preventive care program
that offers up to $100 rewards and other discounts to you and your family when you seek timely screenings and vaccinations, stay active, and learn new skills.
