South Country Health Alliance
South Country Health Alliance (South Country) was formed in 2001 to improve the way our members receive health care. We now have more than 30,000 members and offer 7 different programs to meet the needs of those in our counties. We work closely with health care providers and county public health and social service departments to support wellness. We also offer our members a variety of health promotion programs.
South Country Health Alliance is a county-based purchasing health plan governed through a Joint Powers Agreement between 8 counties.* The Joint Powers Board is comprised of 8 elected county commissioners, one from each county owner. The Joint Powers Board provides the organization’s vision and policy direction.

Community Reinvestment Grants
South Country is inviting proposals for initiatives that will benefit our members and help address the challenges and health disparities of living in rural Minnesota. We will award up to $2.16 million dollars to qualified applicants for this grant process. Respondents may propose grant funding for projects up to 2 years, with awards up to $250,000 for approved projects. Special attention will be given to proposals that focus on areas of behavioral health, homelessness and/or social isolation.
