Medicare Part D: The Medicare prescription drug benefit program. We call this program “Part D” for short. Medicare Part D covers outpatient prescription drugs, vaccines, and some supplies not covered by Medicare Part A or Medicare Part B or Medical Assistance. Our plan includes Medicare Part D.Medicare Part A: The Medicare program that covers most medically necessary hospital, skilled nursing facility, home health, and hospice care.
Medicare Part B: The Medicare program that covers services (such as lab tests, surgeries, and doctor visits) and supplies (such as wheelchairs and walkers) that are medically necessary to treat a disease or condition. Medicare Part B also covers many preventive and screening services.
Medical Assistance: This is the name of Minnesota’s Medicaid program. Medical Assistance is run by the state and is paid for by the state and the federal government. It helps people with limited incomes and resources pay for long-term services and supports and medical costs.
It covers extra services and some drugs not covered by Medicare. Medicaid programs vary from state to state, but most health care costs are covered if you qualify for both Medicare and Medicaid.

Be Active™ Exercise Reward Program

South Country offers the Be Active™ program that rewards members for exercising. South Country will reimburse up to $20 of the monthly health club membership fees for South Country members enrolled in MSC+, SharedCare, or SingleCare; up to $65 of the monthly health club membership fees for AbilityCare; and up to $40 of the monthly health club membership fees for SeniorCare Complete. There is no limit per household, as long as each participant is eligible and enrolled during the respective month and has a paid health club membership. Here’s how it works:

  1. JOIN a participating health club. Click here to find a participating health club in your community (external link). Is your local club not on the list? If not, call Member Services for help nominating your health club for the Be Active™ program.
  2. SIGN UP for the Be Active™ program. Fill out the Be Active™ program enrollment form at your health club. Remember to bring your member ID card with you. Or go to NIHCArewards.org (external link). Under MEMBER options choose First Time Enrollment. Follow the steps to get enrolled. You can also scan the QR code on this page to start to enroll.
  3. EXERCISE! SingleCare, SharedCare and MSC+ members will be reimbursed up to $20 off your monthly membership fee each month with a minimum of 4 visits per month. SeniorCare Complete members will be reimbursed up to $40 off and AbilityCare members will be reimbursed up to $65 off your monthly membership fee each month with no minimum amount of visits. We recommend you record your attendance at a health club for each workout completed.

The member must be enrolled in SeniorCare Complete (MSHO), AbilityCare, MSC+, SharedCare or SingleCare with South Country during the month they exercise at the respective health club. 

 

There is no minimum number of days per month required for SeniorCare Complete (MSHO) and AbilityCare members to exercise at the participating health club. There is a minimum of four (4) days per month required for MSC+, SingleCare and SharedCare members to exercise at the participating health club. 

 

The participating Be Active member must be at least 18 years of age. Members must complete the Be Active™ program enrollment prior to participating. You’ll see your discount 30-60 days after you’ve exercised. You’ll continue to get the discount every month you’re eligible.

The Be Rewarded™ program offers gift card rewards to eligible South Country Health Alliance (South Country) members who complete important preventive care services on time. Find out how you can earn rewards by downloading a voucher from the list below.

Be Buckled™ Free Car Seat Program

South Country will provide one car seat per child age 7 years and younger, per lifetime, along with training on how to safely use the car seat. Call your local county public health office to learn more about this program and to schedule a time to receive your child’s car seat and training. Community Education Discounts

Community Education offers classes for both children and adults. Brush up on special skills or learn a new hobby. South Country will cover up to $15 of the registration fee for most Community Education classes (up to 5 per calendar year). Call your local Community Education office or Member Services for more information.

Embracing Life Online

Embracing Life Online is a companion website for new and expecting mothers to the printed guide and calendar. The website pages have helpful information about prenatal care and care for newborns during their first year of life. Call Member Services to ask for a copy or download it from the Embracing Life Online pages.

The printed prenatal guide and calendar is made especially for new and expecting mothers. It has helpful information about prenatal care and care for newborns during their first year of life. It includes a pregnancy calendar for you to follow from week to week as well as additional online content. If you are pregnant, tell your county eligibility worker. Then call your County Public Health Department or South Country Member Services and ask for a free copy of the Embracing Life prenatal guide and calendar.
