Medicare Part D: The Medicare prescription drug benefit program. We call this program “Part D” for short. Medicare Part D covers outpatient prescription drugs, vaccines, and some supplies not covered by Medicare Part A or Medicare Part B or Medical Assistance. Our plan includes Medicare Part D.Medicare Part A: The Medicare program that covers most medically necessary hospital, skilled nursing facility, home health, and hospice care.
Medicare Part B: The Medicare program that covers services (such as lab tests, surgeries, and doctor visits) and supplies (such as wheelchairs and walkers) that are medically necessary to treat a disease or condition. Medicare Part B also covers many preventive and screening services.
Medical Assistance: This is the name of Minnesota’s Medicaid program. Medical Assistance is run by the state and is paid for by the state and the federal government. It helps people with limited incomes and resources pay for long-term services and supports and medical costs.
It covers extra services and some drugs not covered by Medicare. Medicaid programs vary from state to state, but most health care costs are covered if you qualify for both Medicare and Medicaid.

South Country Health Alliance prides itself in promoting preventive care and staying healthy. Take Charge™ Wellness Programs are one of the ways that South Country empowers its members to be as healthy as they can be. One of these being the BeActive™ , one of the Take Charge!™ Wellness reward programs, which gives members up to a $20 monthly discount on their health club membership fees. Members enrolled in AbilityCare, SharedCare, SingleCare, MSC+ and SeniorCare Complete are eligible for this benefit as long as they are enrolled during the month they exercise. There is no limit per household, as long as each participant is eligible, enrolled during the respective month and has a paid health club membership.

Here are three steps you can take to get your discount.

  1. Join a participating health club. Find a participating health club in your community online at NIHCArewards.org or call Member Services at 866-567-7274, TTY 800-627-3529 or 711. If the local club that you exercise at is not on the list, you can nominate it for the BeActive™ program by calling Member Services for help.
  2. Sign up for the BeActiveTM program. Fill out the BeActive TM program enrollment form at your health club. The only thing you will need is your South Country member ID card. Another option is to go to NIHCHrewards.org. Choose First Time Enrollment under MEMBER options and follow the steps to get enrolled.
  3. Exercise! SingleCare, SharedCare, and MSC+ members will get up to $20 off your monthly membership fee each month with a minimum of four visits to their health club per month. SeniorCare Complete and AbilityCare members will get up to $20 off their membership fee each month with no minimum amount of visits. For each workout completed, we recommend you log your attendance at your health club.

Remember, members must be enrolled with South Country and their participating health club during the month they exercise. Members must complete the BeActiveTM program enrollment form before participating. For every month you are eligible, you will continue to receive your discount. You will see your discount 30-60 days after you have exercised.
